Responsibilities
LPN Job Responsibilities
Licensed practical nurses choose Phoenix Home Care & Hospice for our values and the opportunities we present to our team members. Our LPNs demonstrate compassion and nursing know-how everyday, by:
- Assisting in the development, review, and revision of a care plan
- Providing nursing care in accordance with the physician’s plan of treatment, and individual plan of care, as authorized by the client’s payer source
- Observing, recording, and reporting reaction to treatment and any changes in client’s condition to appropriate personnel and/or physician
- Providing ongoing instruction and supervision of client, family, or other health team personnel in the plan of treatment.
- Participating in education programs, quality assessment activities, nursing meetings, and other staff meetings as required
