As a Licensed Vocational / Practical Nurse you'll contribute to our success in the following ways:

+ Follow each patient's individualized care plan and physician orders.

+ Monitor patient progress and report changes or safety concerns to the care team.

+ Help patients and caregivers learn self-care techniques to support their health.

+ Work with the RN Case Manager to update care plans as patient needs change.

+ Document medication regimens and update medication profiles according to agency guidelines.

+ Communicate with physicians, patients, families, and the care team to ensure coordinated care.

+ Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.

+ Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

+ Current State License as a Licensed Vocational / Practical Nurse

+ Graduate of an accredited Certificate, Diploma, or Associate School of Nursing

+ 1 year of experience in a clinical care setting * Ideally Med/Surg or Critical care*

+ Experience in a hospice or adult/geriatric home health environment is preferred

+ Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

+ Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
