Job Description:

As a Speech-Language Pathologist , you'll contribute to our success in the following ways:

+ Evaluate patients and create personalized speech therapy care plans.

+ Complete initial and ongoing patient assessments following regulatory guidelines.

+ Develop plans to keep patients safe using available family and community resources.

+ Communicate patient progress and changes with physicians and the care team.

+ Deliver speech therapy services based on the patient's care plan and physician orders.

+ Update care plans and request additional visits when needed.

+ Provide education and home therapy exercises to patients and caregivers.

+ Maintain accurate documentation and coordinate care with other healthcare professionals.

+ Ensure therapy notes and visit documentation are completed on time.

+ Work in a physically active role that may include lifting, transferring patients.

What is Required?

+ Master's degree from an accredited educational program in Language Pathology

+ Current, unrestricted SLP license in the state of work

+ Minimum one (1) year of experience as a SLP in a clinical care setting

+ Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

+ Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
