As a Clinical Team Manager , you'll contribute to our success in the following ways:

+ Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are met.

+ Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.

+ Ensure timely start of care and ongoing compliance with applicable regulatory guidelines.

+ Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.

+ Conduct regular OASIS reviews to support accurate documentation and quality patient care.

+ Provide ongoing guidance, coaching, and support to direct care team members and assist with interviewing and hiring clinical staff.

+ Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.

What is Required?

+ Associate degree in a Nursing related field, Bachelor preferred

+ 2 years of home care experience in Home Health or Hospice environment

+ 1 year of supervisory experience required

+ Current, unrestricted RN license valid for the state of work

+ Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office

+ Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

+ Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
